Supporting young people with SEND into adulthood, learning and employment

LIIA’s Post-16, Preparation for Adulthood (PfA), Transitions and NEET work supports London to improve outcomes for young people with SEND as they move into adulthood. This includes strengthening pathways into education, employment, training and independent living, and ensuring transitions between children’s and adult services are timely, coordinated and person-centred.

The programme connects SEND, education, health and social care partners to reduce fragmentation, improve post-16 provision and prevent young people with SEND becoming Not in Education, Employment or Training (NEET).

Project Goals

This project aims to strengthen London’s approach to preparation for adulthood so that young people with SEND experience positive, well-planned transitions into adult life. It focuses on improving coordination between children’s and adult services, embedding PfA across SEND systems, strengthening post-16 pathways and reducing the risk of young people becoming NEET. Through shared learning, diagnostics and system leadership, the programme supports better outcomes in education, employment, health and independence.

Governance

This programme reports into the ALDCS SEND Steering Group and links to post-16, transitions and workforce networks across education, health and social care. It aligns with wider SEND commissioning, inclusion and workforce priorities.
